Discharge Planners

Discharge Planners are available to provide the support to prepare you for a smooth return to your home and community. These planners will visit your room and confidentially discuss your individual needs prior to discharge. They will assist in identifying and obtaining resources, both within the hospital and in the community which may be helpful to you.

Supporting your needs

The Discharge Planners work closely with your physician to identify and address any need you may have upon discharge. We can assist in coordinating Medicaid and Social Security Disability applications, as well as a variety of other needs:

  • Acute Inpatient Rehabilitation
  • Transfer to V.A. Hospitals
  • Pharmaceutical Assistance Programs

A Discharge Planner can be contacted by calling (910) 577-2493 or (910) 577-2345, Monday through Friday during the day. On weekends, holidays and in the evening, assistance is available by calling (910) 577-2345 and asking for the Person On Call for Discharge Planning.
