Onslow Memorial Rehab Center

The Onslow Memorial Rehab Center is the only clinic in the area to treat all adult patients one-on-one (licensed therapist with patient) at every session, and one of few Jacksonville clinics offering comprehensive rehabilitation services in one convenient location. Our disciplinary setting allows therapists to work together to provide the best plan of care for patients—from adults to children of all ages.

We are proud to offer longer, 45-minute sessions with individualized care to every one of our patients. And as an affiliate of Onslow Memorial Hospital, we are able to treat all ages and diagnoses with a unique multidisciplinary approach to rehabilitation.

Physical Therapy

Specialty Sports Medicine Rehab, Orthopedics, Vestibular Therapy, Pediatric Therapy, Joint Replacements, Stroke Rehab, Amputee Management, Chronic Pain, Work-Related Injuries, Lymphedema ManagementNeuro Rehab, and General Medical Issues.

Occupational Therapy

Hand Therapy, Custom Splint Fabrication, Stroke Rehab, Ergonomic and/or Industrial Rehab Assessments, Treatment for Work-Related Injuries, Pediatrics, and Neuro Rehab.

Speech Therapy

Dysphagia Management, Videofluoroscopic Swallow Studies, Cognitive-Linguistic and Speech Therapy, Voice and Stuttering Therapy, Bells Palsy Therapy, Pediatric Therapy.

Cardiac Rehabilitation

In addition to our Rehab Center, cardiac rehab inpatient services are provided on the 1st floor in the hospital and include supervised exercise programs, education on nutrition, and recommendations for lifestyle changes. For more information on cardiac rehab services, click here.

Inpatient services are also provided at Onslow Memorial Hospital by a multidisciplinary team of experienced physical and occupational therapists and speech-language pathologists who are able to provide evaluations and treatment appropriate for admitting diagnoses. Therapy in this setting consists of therapeutic exercise, functional activity as indicated, patient and family education, ongoing assessment and supportive care as needed. Rehab services are provided to patients in all areas of the hospital as prescribed by their physicians, to include infants in the Neonatal Intensive Care Unit. Therapists work in collaboration with physicians, nursing staff, families, discharge planners and dietary staff to make recommendations, to ensure safety, and to facilitate our patients' return to their former living environment as soon as possible.
