March is Social Work Aware­ness Month

March is Social Work Awareness Month, a time to recognize and celebrate the vital role social workers play in healthcare. At Onslow Memorial Hospital, social workers, or case managers, are an essential part of the care team, supporting patients, families, and caregivers during some of life’s most challenging moments.

The Case Management team consists of Bachelor’s, Master’s, and Licensed Prepared Social Workers (LPSW). This team is skilled at accessing for social determinants of health and health related social needs.

Hospital social workers serve as advocates, problem-solvers, and compassionate listeners. They help patients navigate complex healthcare systems while addressing emotional, social, and practical needs that can impact recovery and overall well-being. From the moment a patient enters our care to long after discharge, social workers help ensure no one has to face their healthcare journey alone.

The Case Management team supports patients across many areas of the hospital, including inpatient units, emergency care, and oncology. Their work may include:

  • Assisting with discharge planning and transitions of care
  • Connecting patients and families to community resources and support services
  • Providing emotional support during illness, injury, or crisis
  • Assisting patients and families in coping with fear, stress, grief, or a difficult diagnosis
  • Helping patients navigate insurance, financial concerns, and advance care planning
  • Supporting families during end-of-life care and difficult decision-making

Social workers also collaborate closely with physicians, nurses, case managers, and other care team members to address barriers that may affect a patient’s health such as housing insecurity, transportation challenges, food access, or caregiver support.

This month, we recognize the dedication, compassion, and professionalism of our social workers. Their work often happens behind the scenes, but its impact is felt every day by the patients and families they serve. Happy Social Work Awareness Month.
