The DAISY Award for Extraordinary Nurses Nomination Form

The DAISY Award is an international recognition program started in memory of J. Patrick Barnes. Pat’s family experienced first-hand the difference his nurses made in his care through clinical excellence and outstanding compassionate care. The family created The DAISY Award to express gratitude to nurses and to enable other patients, families, and staff to thank and honor their special nurses.

The PETALS Award Nomination Form

The PETALS Award is a new program administered by the Division of Nursing to thank those who partner with nurses in caring for our patients and families. It was developed as a complementary program to the DAISY Award, which recognizes nurses for providing exceptional patient care. Patients, family members and other staff members can nominate staff for the PETALS Award.

The DAISY & PETALS Team Award Nomination Form

The DAISY Team Award is a concept brought to us by DAISY Award Partner Thomas Jefferson University Hospital in Philadelphia. It recognizes that while an idea to achieve better patient and family outcomes may start with one individual, it often takes an entire team to implement successfully. The DAISY Team Award is designed to honor collaboration by two or more people, led by a nurse, who identify and meet patient and/or patient family needs by going above and beyond the traditional role of Nursing. The PETALS Team Award is a complementary program to the DAISY Team Award, which recognizes teams who partner with our nursing staff who have come together to solve a specific situation by going above and beyond.
