Onslow Memorial Hospital Discharge Planning Department
During your stay at Onslow Memorial Hospital, Discharge Planners are available to prepare you and your family for a smooth return to your home and community. We will assure that you have the proper support you need upon discharge from the hospital.
The Discharge Planners will visit your room and confidentially discuss the individual needs upon the patient's discharge. We will assist in identifying and obtaining resources, both within the hospital and in the community which may be helpful to you.

The Discharge Planners work closely with your Physician to identify and address any need you may have upon discharge. We can assist in coordinating Medicaid and Social Security Disability applications, as well as a variety of other needs:
- Acute Inpatient Rehabilitation
- Transfer to V.A. Hospitals
- Pharmaceutical Assistance Programs

Discharge Planning is available on weekends, evenings, and Holidays by calling 577-2345 and asking for the person On Call for Discharge Planning.
